After a hospital or rehab stay, many older adults come home weaker than anyone expected. Doctors call it deconditioning — the muscle, balance, and stamina lost from lying still. It's incredibly common, and it's reversible with the right, steady support.

The hard part is the gap: insurance-covered physical therapy often ends after a handful of visits, and the family is left wondering how to keep the progress going. That's exactly where we come in — in the home, on a schedule that fits the day, for as long as it helps.

Meet them where they are

We start with what's possible today — even if that's edge-of-bed movement or standing with support — and build from there.

Rebuild everyday strength

Gentle, progressive work aimed at the movements that matter: standing up, sitting down, walking to the bathroom, climbing a step.

Restore confidence

Fear of falling keeps people from moving, which makes them weaker. We rebuild steadiness and trust in the body alongside the strength.

Work with your care team

We coordinate with the discharge plan and any home-health or PT still involved — we complement that care, never replace it.
